Fellowship Application Criteria

Applications are accepted from any physician with primary care specialty training, including family medicine, internal medicine, pediatrics, med-peds, physical medicine and rehabilitation, and emergency medicine.

An applicant should possess the following qualifications:

  • Robust sports medicine experience with team physician coverage during residency
  • Strong grades and board scores
  • Keen interest and ability in teaching
  • Good organizational skills
  • Independent learner
  • Motivated to make the most of work and recreation time
  • Sorry, we do not sponsor any visas

Because we participate in the National Resident Matching Program (NRMP), applicants must submit application and supporting documents for the Sports Medicine Fellowship online through the Electronic Resident Application Service (ERAS). Applications are accepted from the opening of the ERAS process to October 15. The Fellowship Selection Committee will review all applications and will contact qualified applicants to schedule an interview. Interviews will take place between September and November.

In support of your application, we must be in receipt in all of the following:

  • Letter of recommendation from your residency director (eligibility for board certification must be confirmed by the beginning of the fellowship year)
  • Two letters of recommendation from physicians who have supervised you in a clinical setting (should be sent directly to the address below)
  • Copy of medical school diploma
  • Copy of medical school transcripts
  • Certificate (or other validation) of all previous training
  • Present state medical licenses (must have Florida medical license by July 1 of fellowship year)
  • Copy of USMLE Step 1, Step 2 (CK and CS), Step 3 or COMLEX Level 1, Level 2-CE and Level 3 Board score reports
  • Curriculum vitae
  • Written personal statement
  • Recent photograph
  • Copy of Educational Commission for Foreign Medical Graduates certificate, if applicable
