What can I do with Bayfront Health MyChart?
Bayfront Health MyChart offers patients personalized and secure online access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. With Bayfront Health MyChart, you can use your smart phone, tablet or computer to:
- Schedule and request medical appointments.
- Check in for appointments online.
- View your health summary from the Bayfront Health MyChart electronic health record.
- View test results.
- Request prescription renewals.
- Conduct video visits or E-Visits with your provider.
- Access trusted health information resources.
- Pay bills and request financial assistance.
- Communicate electronically and securely with your medical care team.
- And more!
Is there a fee to use Bayfront Health MyChart?
Bayfront Health MyChart is a free service offered to our patients.
ACCESS
How do I sign up?
Patients who wish to participate will be issued an Bayfront Health MyChart activation code during their clinic visit or hospital stay. This code will enable you to log in and create your own username and password.
If you do not have an activation code and are not able to complete the ‘Sign up now’ process, then you can fill out this form.
- Download the Authorization form
- For Format of Records select ‘Patient Portal’
- Send the completed form and a copy of your driver’s license/ photo ID to MyChartSupport@BayfrontStPete.com
Who do I contact if I have further questions?
You may e-mail us at MyChartSupport@BayfrontStPete.com, or you can call our Bayfront Health MyChart Patient Support Line at (321) 843-7759, Monday – Friday, between 8:00 am and 6:30 pm. The Bayfront Health MyChart Patient Support line can assist you with password resets, login issues, questions about the app and site navigation.
I forgot my password. What should I do?
You may contact our Bayfront Health MyChart Patient Support Line at (321) 843-7759, Monday – Friday, between 8:00 am and 6:30 pm to request a new, secure password. You may also click the "Forgot password" link on the sign-in page to reset your password online.
Can you send me a new activation code as I have lost it, let it expire or did not receive it?

What do I need to use Bayfront Health MyChart?
You can access Bayfront Health MyChart on your smartphone or tablet via the MyChart mobile application, or via an up-to-date internet browser on your smartphone or computer (such as Chrome, Firefox, Safari or Microsoft Edge).
My activation code does not work, what should I do?
For your security, your activation code or link expires after a set amount of time depending on how you received it.
- If you received an email or text message from us with a link to sign up for MyChart, that link is valid for 5 minutes. After the 5 minutes, you can still use the link, but you will need to enter more information about yourself to verify your identity.
- If you received a letter, after-visit summary, or billing statement with an activation code, that code expires after 30 days and is no longer valid after the first time you use it.

I have a MyChart account from another organization. Do I need to create one for Bayfront Health?
Yes, you will need to create a Bayfront Health MyChart account (you can use the same username and password if you want to). Once you have created a Bayfront Health MyChart account, you can link it to your other account to see your information in one place. See below for instructions on linking MyChart accounts.
Can I link my Bayfront Health MyChart to one from another facility?
Yes, you are able to link your MyChart accounts.
Steps:
- Go to ‘Your Menu’
- Select ‘Link My Accounts’
- Enter the name of the facility you want to link
- Select ‘Link account’
- Enter the Username and Password for the MyChart you are linking
I have MyChart accounts for multiple facilities. Can I use one for everything?
You can view your clinical data, send messages, and view and schedule appointments for all of the facilities that you have linked.
Some of the questions asked during the Self Sign-up seem intrusive. Why?
These questions are being asked by Experian, the third party vendor that is used to verify your identity. We use Experian to confirm your identity to ensure the access to your records are secure.

How is Bayfront Health MyChart secure?
We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames and passwords. Each person controls their password, and the account cannot be accessed without that password. Unlike conventional e-mail, all Bayfront Health MyChart messaging is done while you are securely logged on to our website.
What is your Privacy Policy?
Bayfront Health MyChart is owned and operated by Epic Systems Corporation and is fully compliant with federal and state laws pertaining to your privacy. Your name and e-mail address will be treated with the same care and privacy given your health records and will never be sold or leased.
I was logged out of Bayfront Health MyChart, what happened?
We aim to protect the privacy and security of your information. While logged into Bayfront Health MyChart, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out. We recommend that you log out of your session if you need to leave your computer for even a short period of time.
COMMUNICATE
If I send a message to my doctor or nurse, when can I expect a reply?
You will generally receive an answer within 1-3 business days. Please note that Bayfront Health MyChart should not be used for urgent situations. Please call your medical provider's office if the situation requires immediate attention or dial 911 if it is an emergency.
Is Bayfront Health MyChart the only way to communicate with my doctor or access my patient record?
No! All your favorite ways of communicating with Bayfront Health will remain. Bayfront Health MyChart is a new complimentary resource added for your convenience to engage with your doctor and Bayfront Health.
MY INFORMATION & RESULTS
When can I see my test results in Bayfront Health MyChart?
Your test results are released to your Bayfront Health MyChart account after your physician has reviewed them. This is generally within 1-2 days.
Why are certain test results not shared electronically via Bayfront Health MyChart?
Your provider is able to determine which types of test results can be accessed through Bayfront Health MyChart. Further, tests of a very sensitive nature are not released.
If some of my health information on Bayfront Health MyChart is not correct, what should I do?
Your Bayfront Health MyChart information comes directly from your electronic medical record at your doctor's office. Ask your doctor to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record each visit. You may also submit a chart correction request to have the information reviewed by our Health Information Management team (Medical Records) within MyChart.
Will any of the clinical information from my CareNotify™ portal be available in MyChart?
Yes, we are in the process of adding your past information into our new electronic health record. When this process is complete, you will be able to view your Results from 2018* forward in MyChart. In addition, your previous Health Issues, Allergies, Medications and Immunizations will be transferred but will need to be reviewed with your healthcare provider on your next visit before being available for you to view in your MyChart.
*Certain Results prior to 2018 may be available.
Where can I update my personal information (e.g., home address, e-mail or change my password)?
Log in to Bayfront Health MyChart and from the left menu, go to the "Preferences" section and select the appropriate option.
FAMILY
Can I view a family member's health record in Bayfront Health MyChart?
Yes, you can. This is called proxy access and allows a patient to log in to their personal Bayfront Health MyChart account and then grant access to a family member. To do this, the patient needs to log into their Bayfront Health MyChart account and search for “Share My Record.” From there they will select “Friends and Family Access” to add you. A second way to get proxy access is for a patient and family member to complete a Proxy Authorization form and return it to one of our medical facilities.
Will I be able to see all of my child's info once I have proxy access to my child's account?
The proxy access you have will change based on the age of your child.
- Birth - 11-years-old: You will have full access.
- 12 - 17-years-old: Your proxy access will continue but the system will automatically restrict certain information to you based on Florida law.
- 18-years-old: The system will automatically remove your proxy access.
How do I get access to my child's Bayfront Health MyChart account?
There are multiple ways for you to gain proxy access to your child's Bayfront Health MyChart account.
- Option 1: If your child is 12 years of age or older and they have their own Bayfront Health MyChart account, they can grant you access from within "Friends and Family Access".
- Option 2: During the next visit for your child, you can sign the Proxy Authorization form and the team member will grant you proxy access.
- Option 3: You can download the Proxy Authorization form and send the completed form and photo identification to MyChartSupport@BayfrontStPete.com
Can I ask questions regarding a family member from my Bayfront Health MyChart account?
No, you cannot ask through your own account, but you can message on behalf of a family member if you have proxy access to their account.
Can my spouse and I share one Bayfront Health MyChart account?
No, due to the sensitive nature of medical information, each adult must establish their own Bayfront Health MyChart account. If a patient chooses to share medical information with their spouse, they can grant them proxy access.

Can I pay a bill thru MyChart?
Yes, you can use MyChart to make a payment for those areas that have switched to MyChart. To see the list of facilities using MyChart, visit Bayfront Health MyChart
To make a payment or set up payment arrangements using MyChart go to Menu / Billing Summary.
If you would prefer to make a payment as a Guest, use MyChart - Pay as Guest.
