Patient Information
To best serve our patients in a safe and timely manner, Orlando Health Bayfront Hospital follows stringent admission policies for our inpatient and outpatient facilities.
Inpatient Admissions
All patients admitted to the hospital overnight will need to check in at the admitting office, located in the front lobby. This applies even if you pre-registered in advance over the phone.
You will need to have these items to complete your registration:
- Identification Information — You will need to provide us with a driver’s license or photo ID, address and telephone number, and your social security number.
- Insurance Information — If you have an insurance card, please bring it with you. To bill your insurance, we will need the name and mailing address of your insurance provider, your policy’s group number and your personal identification number. If your insurance requires a referral, please bring the form with you to your first appointment. If you are covered by Medicare or Medicaid, please bring the appropriate cards.
- Doctor’s Orders and/or Medical History (unless ER patient) — Bring any written or printed paperwork from your doctor. If you are coming in for a second opinion or on a physician referral, you may need to bring medical records, X-rays, lab results, pathology slides or other items. Your doctor already may have sent these to us, but be sure to check with them before your appointment.
- Advance Directive — If you have an advance directive (such as a living will and/or durable power of attorney for healthcare), please be sure to bring it with you.
- If you have an extended stay, you can bring the following items with you:
- Personal toiletries, bathrobe, slippers or socks
- Any medications you routinely take with your name written on the containers
- A list of all your current prescriptions
- Eyeglasses, dentures, hearing aids, etc.
- Electrical appliances (e.g., hair dryer, electric shaver, radio)
Important Reminders:
- We cannot be held responsible for personal items that are lost or stolen on our premises. We ask that you leave all valuables at home or with a family member when you are admitted, such as:
- Jewelry
- Money
Credit cards
- We will need to confirm your address and make copies of your identification and insurance cards any time you are admitted to the hospital.
- Copays or deductibles are due at the time you receive services. A financial counselor is available for any billing or insurance questions.
Outpatient Admissions
To register for and schedule an outpatient test or procedure ordered by your doctor (such as mammogram, blood test, chest X-ray), call central scheduling at (727) 893-6044. You will be pre-registered, scheduled for an appointment date and time, and provided with any necessary instructions.
Bring the following with you on the day of your procedure or test:
- Identification Information — You will need to provide us with a driver’s license or photo ID, address and telephone number, and your social security number.
- Insurance Information — If you have an insurance card, please bring it with you. To bill your insurance, we will need the name and mailing address of your insurance provider, your policy’s group number and your personal identification number. If your insurance requires a referral, bring the form with you to your first appointment. If you are covered by Medicare or Medicaid, please bring the appropriate cards.
- Doctor’s Orders — Bring the outpatient prescription you received from your doctor.
