Nominate a Nurse
The DAISY Award for Extraordinary Nurses® offers a special way for patients and patient families to thank nurses whose care has had a meaningful impact on their lives. The award is part of the DAISY Foundation’s mission to recognize nurses for the compassionate and skillful care they provide patients and families.
What is the DAISY Foundation?
The DAISY Foundation is a not-for-profit organization created in 1999 in memory of J. Patrick Barnes by his family. Patrick died at age 33 from complications of idiopathic thrombocytopenic purpura (ITP), an autoimmune disease. Through the award, which is an acronym for Diseases Attacking the Immune SYstem, the family wanted a way to recognize and thank nurses for the extraordinary care they give their patients and families on a daily basis, just as they had experienced with Patrick.
How to Nominate an Extraordinary Nurse
Nominations for The DAISY award can be made by patients, families, and colleagues. Award recipients are then chosen by a committee at Orlando Health Bayfront Hospital and are recognized at a special presentation attended by the honoree’s fellow team members as well as patients and visitors. Share your story below by completing the online form to nominate a nurse who made a difference in your care.
